We Are With You Every Step of the Process

Getting started with our team at Hospice Specialists of Louisiana is easy. Depending upon where you or your loved one is in the hospice progression, we can help.

If you have been referred to hospice by your doctor or hospital:

The next step is for us to meet with you and a family member(s) so we can provide you with an Informational Visit. One of our Patient Care Coordinators will meet with you in person at your home or residence and answer all questions you and your family may have about hospice, and let you get to know us. Choosing the right hospice provider is an important decision, and we want you to be fully comfortable with your choice. Our Patient Care Coordinator will also obtain a Release of Information Form from you so that we can obtain your medical records.
Next, after the Informational Visit we will then review all relevant medical records and orders. Assuming you qualify for hospice in your medical records, we will then meet again to perform a Needs Assessment and Admission. During the Admission and Needs Assessment we are determining what medical equipment may be needed to help you or your loved one. This can include everything from bed pans to hospital beds and wheelchairs. Our Team takes care of ordering all this equipment for you.
Once you are ‘Admitted’ on hospice, we then provide care through our Registered Nurses, Licensed Practical Nurses, Certified Nursing Assistants, Social Workers, and Home Health Aids. Our Team Chaplain is also available to you if you request.
All of the above can take place in as little as a day or two in some cases. If additional information is needed from your doctor, the process may take a few days. During this time, we are available to you and your family to answer any questions you may have.
